First Consultation Appointment

• First-time appointments are only accepted through medical institutions or referring physicians.
• Please send the following medical application form and referral letter to
  (Regional Liaison Office) by email.
• The referral letter must be written in Japanese or English.
• We will contact you at a later date after checking the contents.
  If we determine that it is acceptable, we will inform you the date and time of the reservation and an approximate estimate of the cost.
  We will also send you a reservation slip (please print it out and bring it with you on the day).
